自主回報疫調系統是什麼？
由於疫調需耗費大量的人力資源，為了降低醫療成本，疾管屬在5/1推出自主回報疫調系統，目的就是希望確診的個案可以自行回報，以利相關單位掌握疫情狀況。
當民眾有疑似COVID-19的症狀時，會前往醫院或採檢站進行採檢，在採檢的同時，醫院或採檢站會留下民眾的手機號碼，如果最終PCR採檢的結果為「陽性」，就會透過簡訊發送「自主回報疫調系統」的網址讓民眾填寫。
疫調系統簡訊內容：
- 簡訊門號：0911514588
- 簡訊文字：「您為 COVID-1 9確診個案，請前往自主回報系統網站，並核對個人資料後，主動回報密切接觸者的資料，並通知您工作或就學聯絡窗口。」
- 系統網址：https://bbs.cdc.gov.tw/
自主回報疫調系統怎麼填寫？
收到指揮中心的簡訊，點擊簡訊上的網址連結，就可開始填寫自主回報疫調系統：
填寫重點
- 基本資料填寫
- 最早出現症狀日期或檢驗陽性日期
- 密切接觸者
- 是否有慢性疾病或懷孕
步驟一、確認資料
如果是確診者，進入網址後需要先輸入「身分證號碼」以及「居留證」後六碼，並且確認系統顯示的資料是否與本人相符。
步驟二、確認診狀
詳細閱讀並同意個資相關的法源規範後，系統會顯示一些確診的相關症狀，若有符合的話請聯繫當地的衛生局人員。
步驟三、確認身分
若身分為孕婦、血液透析（洗腎）、年齡≧65歲獨居者等高風險族群，若不是則繼續填答，然後回想發病前兩日的密切接觸者有哪些人。
步驟四、填寫疫調單
最後填寫完成疫調單，內容包含手機號碼、居住地址、職業身分、公司或學校，若確診者年紀比較小，也有法定代理人的欄位可以填寫，以利指揮中心聯繫、追蹤。
步驟五、填寫完畢
填寫完畢後，自主回報疫調系統會顯示衛教相關的資訊，請確實讀完，並且民眾也可以使用「台灣社交距離APP」上傳自己的隨機ID，這樣曾經與你有接觸過的人也會收到通知。
透過「健保快易通」APP進入疫調系統
假如沒有收到疫調系統的簡訊，但經由PCR採檢為陽性的確診者，也可以透過「健保快易通」app進入自主疫調系統網站進行回報。
